About this template
A living medical record for your whole family. Drop in PDFs, photos of prescriptions, and scanned lab reports — Plank runs OCR, identifies who each document is about, and files the extracted data into a curated wiki page for that person. Lab values, diagnoses, medications, doctor notes, allergies, and chronic conditions all stay current and source-backed. The wiki is person-first: each family member has a rich profile, a full lab history with trends, and a registry of every source document — never a brittle summary, always the full picture.
